Funeral Wishes Form

As part of our Lent series there is a session including how to plan a funeral.

If you would like to let the church office and clergy know your funeral wishes to help your loved ones know your wishes when the time comes, please complete this form and we will keep it on file.

Note About Service Location: You are free to choose where you have a service and which funeral home you would like to work with. The form gives the most typical options that we have found people choose in Maple Ridge for a service location and we are only aware of two funeral chapels at this time. We are very happy to work with any funeral directors, and if you would like a service outside of Maple Ridge you can also note that down on the form. For any alternatives please choose Elsewhere and make a note at the end of the form.
